Perioperative Pathways Leading to Better Outcomes

چکیده
A n enhanced recovery program (ERP) is a set of multimodal, multidisciplinary interventions that are geared toward mitigating the undesirable effects of the surgical stress response.1 The purpose of an ERP is to accelerate postoperative surgical recovery by attenuating the factors that lead to surgical stress. Successful execution of an ERP requires coordination of the multidisciplinary teams, including surgery, anesthesiology, nursing, nutrition, physical therapy, and hospital team members.1 Together, these individuals combine best practices, namely, organization of care and effective clinical management in an evidenced-based framework. Other terminologies have been used to describe this coordination of care. These include enhanced recovery after surgery (ERAS), enhanced recovery pathways, and perioperative surgical home.1 Although there are some variations in focus, the main objectives of these practices are to improve patient care and potentially reduce health care costs.1 Colorectal surgery is among the earliest surgical models to which ERP principles were applied.1 The various elements of an ERP include preoperative patient education, modified preoperative fasting guidelines, preoperative medical optimization, fluid management, the role of bowel preparation, early oral intake, early ambulation, and standardized multimodal analgesic regimens. Specifically, 4 areas of patient care—preoperative medical optimization, pain management, use of bowel Enhanced Recovery: Perioperative Pathways Leading to Better Outcomes

Protocols for Management of Underserved Patients Undergoing Arthroplasty: A National Survey of Safety Net Hospitals
Background: Although it has been shown that perioperative protocols enhance arthroplasty care and safety, it isnot known how prevalent their use is in safety net hospitals, which operate with a mandate to treat the poor andunderserved. Understanding the elements currently included in standard perioperative arthroplasty protocols at variousinstitutions may help guide future interventions and pol...
متن کاملEffect of Perioperative Management on Outcome of Patients after Craniosynostosis Surgery
BACKGROUND Craniosynostosis results from premature closure of one or more cranial sutures, leading to deformed calvaria and craniofacial skeleton at birth. Postoperative complications and outcome in intensive care unit (ICU) is related to surgical method and perioperative management. This study determined the perioperative risk factors, which affect outcome of patients after craniosynostosis...
متن کاملSystematic review of perioperative and quality-of-life outcomes following surgical management of localised renal cancer.
CONTEXT For the treatment of localised renal cell carcinoma (RCC), uncertainties remain over the perioperative and quality-of-life (QoL) outcomes for the many different surgical techniques and approaches of nephrectomy. Controversy also remains on whether newer minimally invasive nephron-sparing interventions offer better QoL and perioperative outcomes, and whether adrenalectomy and lymphadenec...
متن کاملUpdate on Techniques for Neuroprotection during Hypothermic Arrest
Surgical repair of the ascending aorta and aortic arch that employs cardiopulmonary bypass (CPB), often with the use of deep hypothermic circulatory arrest (DHCA), allows for lifesaving therapy. In doing so, however, it also represents a unique injury paradigm leading to derangements in numerous homeostatic pathways. Organ injury, most notably cerebral injury, may result as a consequence of the...
متن کاملPrevention of unplanned perioperative hypothermia.
Hypothermia is one of the most common complications experienced by surgical patients. Better postoperative patient outcomes are achieved when normothermia is maintained. Perioperative nurses should understand how to maintain normothermia, the causes of hypothermia, and adverse patient outcomes that result from hypothermia. Nursing interventions to help prevent hypothermia can be implemented dur...
